All city forms included in this section may be completed and mailed to City of Birchwood Village, 207 Birchwood Ave, Birchwood, MN 55110. You may also e-mail a completed copy of the form to info@cityofbirchwood.com
The complaint form will be acted upon only if it includes the name/address of the person completing this form.
The dog license application must include payment (checks made payable to the City of Birchwood Village) and copy of vaccination record. The tags will be sent upon receipt of the above items.
